    I was reading the thread about myectomy vs. ablation and wanted some advice.

    I had a myectomy done 12/1998.

    Here are my numbers since:

    Date/Gradient/Septal thickness





    The October 2000 echo was done while adjusting my pacer, so the numbers are in ranges. Does this make sense you? Can my septal thickness decrease like that? Should I consider an ablation with the blockage I have. I take 100mg of Lopressor twice a day and that's all. My pacer doesn't trigger unless I go under 50bpm. My cardiologist wants me to get an ICD for precautionary reasons. He mentioned the ablation once or twice, but has since dropped it. Any advice? I have my next appointment in mid-September and wanted to get a jump on what to ask.

    The risk factors that are used as a guideline for ICD placement are these (most doctors will want you to have several risk factors unless you have a lot of vtach or a septum over 3 are sometimes enough by themselves):

    family history of sudden death, personal history of fainting, ventricular tachycardia, adverse (incorrect) blood pressure response to a stress test and a septal wall thickness of 3 or more.

    There are some known incidences of hypertrophies "reverting" or reducing after a long time on beta-blockers but 1.1 to 1.7 seems like a big jump.

    Are you seeing an HCM specialist? If not, I would highly recommend it. I can't speak to your gradient here, but I think Lisa can --and will.

    First ablations should not be done with septal measurments less than 1.6. If your numbers are correct and your current measurment is 1.1 it appears that there is more going on here.

    I am rather confused by these numbers, further where did you have your myectomy as it does not seem that the results were very good.

    I would much rather talk about this one then run back and forth on the board...
