  • HCM vc HOCM

    Can anyone tell me what causes some people to have obstructive HCM? If everyone with hcm has a thickened heart, what causes some people to become obstructed? A simple question but I am sure not a simple answer. Also, does everyone that has an obstruction have a fast, hard, dynamic heart rate? Do people that do not have obstruction have this problem?

    Dear Kathy,

    Nope, no one really knows why some people become obstructive and some don't. It may turn out to be a particular genetic mutation, but that is merely a guess on my part.

    I don't have an obstruction, but I certainly have lots of tachycardia and a very hard heart beat. I'm sure that is true for a lot of us.

      Even among those with obstruction, the degree of obstruction varies with many people. I wish we did know why!

        Kathy,

        Ditto. Nobody really knows. My septal thickness is only 1.5cm, but i have a large obstruction (100mm gradient), while others can have much larger septums and no obstruction at all. It just happens i guess

          There is no answer that I have ever heard from any med. prof that clearly explains WHY...
          Ideas - the shape of the Mitral Valve? the pressures in the heart? Genetics? the moon..only kidding on that one

          lost: 5 family members to HCM (SCD, Stroke, CHF)
          Others diagnosed living with HCM (or gene +) include - daughter, niece, nephew, cousin, sister and many many friends!
          Therapy - ICD (implanted 97, 01, 04 and 11, medication
          Currently not obstructed
          Complications - unnecessary pacemaker and stroke (unrelated to each other)

            Dr Shah called me yesterday and I asked him this question. He said it was a long answer and that he would tell me about it the next time I saw him (probably a long time from now! LOL). He actually thinks he knows why and he said he was going to Peru soon and giving a talk on this same topic. I wish I could go and hear what he has to say! I asked him if there would be a paper on this and he didn't expect to be doing one. What a shame! Kathy
