    Golf Sponsorship Options

    Here is a section from the Brochure that was just mailed out for the Golf Outing. If you or anyone you know would like to help by sponsoring a hole or an ad in the program book, please contact the HCMA or drop me a note.

    SPONSORSHIP OPTIONS
    1. HCMA Benefactor – Receive a foursome of golf at the HCMA golf outing, special recognition in the program, one full hole sponsorship with full page ad, banner at 1st or 18th hole and 1 year of Benefactor status in the HCMA National Newsletter: $1250.00 ($1000.00 without golf)
    2. HCMA Patron – Receive a foursome of golf at the HCMA golf outing, special recognition in the program, one full hole sponsorship with full page ad, and 1 year of patron status in the HCMA National Newsletter: $850.00 ($500.00 without golf)
    3. Full Hole Sponsorship with full page ad in the program and foursome of golf at the HCMA golf outing: $700 ($275.00 without golf)
    4. ˝ Hole Sponsorship with ˝ page ad in the program: $150.00
    5. ˝ Page ad in the program: $100.00.
    6. Ľ Page ad in the program: $50.00
    7. Coupon ad option: Turn your ad into a coupon. Example: “Bring this Ad in for a 20% discount on merchandise”
    8. Friends List – Name and one line message: $25.00 – i.e. The Smith Family supports the HCMA in their efforts to raise awareness of HCM.

    Remember, the more support we get for the golf outing, the more support we can give to those with HCM!!!

        Dx age 12 current age 46 and counting!
        lost: 5 family members to HCM (SCD, Stroke, CHF)
        Others diagnosed living with HCM (or gene +) include - daughter, niece, nephew, cousin, sister and many many friends!
        Therapy - ICD (implanted 97, 01, 04 and 11, medication
        Currently not obstructed
        Complications - unnecessary pacemaker and stroke (unrelated to each other)
